Vending machine: If your scalpel is not sharp enough, you can't. There is a mechanism for doing so in the present, near a microwave.

But I can tell this is your first trip in time, so don't worry about that. You can actually go down to the clinic entrance without stepping out of time. There's a reason the game lets you do this. Look around.
