I drum my fingers any and every way: thumb to pinky, pinky to thumb, middle-index-ring-pinky-thumb... whatever. I put "other," since there wasn't an "all of the above" option.

I nearly always do include the thumb, though, in whatever pattern I use. That's because I'm compulsive about my fingers: if some of my fingers make contact with some object, all the others have to too. To drum my fingers but not my thumb would drive me nuts.
