POA -Possibly a Glitch in My Game??

Hi, My question is whether an item is ever removed from inventory without using it in the game once it's been found and added? Here's my current inventory:
3 Nanoxite Stones
/ I.D. Card
/ Business Card
/ Metal Rod
/ Plastic Cup
/ 2 Peryolitium Shards
/ Passcard (CQ30021)
/ Crowbar
/ Pickaxe
/ Round Pot
/ 1 Dynamite Stick

/ ISO Officer's Cap
/ Small Mirror
/ Leather Briefcase
/ Supply Room Card
/ Radio Receiver
/ Pet Snacks

Went back to Caves with 3 N'stones and found one more along with a P. Added both to inventory and went looking for the lost (rescued) miner. Suddenly only 3 N'stones. What happened??? Thanks for setting me wise on what I did wrong. Nora
