This has happened to me too. What goes wrong is that you have to open a chest to get the items, including the key. If you mis-click and don't open the chest, you don't get the key. Also, sometimes the chest is hootery-trapped (Sam's electronic censor refused the term as it was originally) with a teleporter, and you get teleported away without being able to grab the contents of the chest. When you go back to the site of the battle, there's nothing there for you to find (with one exception that would require a spoiler). If you got no items at all, this could be what happened.

Keys are always the first item a party finds. You'll notice that whenever you find a key and a pile of other stuff, the key is listed first among the erstwhile empty spaces in your character's inventory.
