POA..Wrench..Far into game, but still can't get it!

I have read through all the posts and just don't get it. I have done so much more then others who have the wrench but still can't get it off that smoking fool. I've been to the pharmacy but don't have an option to use said item. What am I missing or doing wrong?? Really could use that
