Murkon's undocumented features

For some reason, when I cast CHAOS sometimes it misses out on a monster. I don't mean that the monster is "not affected", it doesn't even show up in the list of what happens to all the monsters. The same happens now and again when I invoke a Rod of Catastrophe.

I only noticed this a couple times because a Gelatinous Cube was conspicuously absent from the list. They only come one at a time, and you never lose count of whether you've hit one or zero of them. However, I've since seen it with other monsters as well.

I assume this is an "undocumented feature"?
