Here's what I do when I'm absolutely desperate to find an Item of Specialness, and I know what level it's on. Cast MAP and methodically go through the entire accessible part of the level, and make sure you've been everywhere. Actually, you can do that with just BLAZE. Just make sure you hit every square.

I know it's there, cause I've found it twice.
